The Anticpatory Care Calendar (ACC) is a free resource for use in any social care setting supporting people with learning disabilities, dementia and would work well with patients and people who are likely to have reduced capacity and or communication difficulties such as frail elderly, stroke or degenerative conditions.
This tool will provide an improved way of recording health issues and can offer solutions to communicate these issues to the appropriate health professionals. It should also facilitate access to health services more quickly.
The ACC is a simple tool to improve the daily surveillance of health. It overcomes some of the barriers that can prevent people from accessing health services effectively.
It provides an innovative approach to assessing health on a daily basis and is designed to alert staff to changes in a person’s health status and provides clear directions about accessing care. A 'traffic light' system triggers the need to respond to changes to the person’s health through observation. A key benefit is that it supports and empowers social care staff to develop a high standard of health record keeping, monitoring health and ensuring people access NHS Cancer Screening Programmes, where appropriate.
Our e-learning site
The purpose of our e-learning site is to allow you to register to implement the ACC within your organisation or care setting. You will also be able to access up to date information regarding the ACC and download the documents and tools and access the e-learning package.
Why register?
Registration is compulsory if you are intending to use the ACC. Health Innovation North West Coast holds the copyright for this initiative. By registering, your organisation agrees to follow the terms governing the use of the ACC.
How does an organisation go about implementing the ACC – what is the process?
- A manager/lead person will register interest on the Health Innovation North West Coast website using the electronic registration form
- The organisation participating in the use of the ACC will agree to use the tool as it is intended under terms & conditions
- Managers will commit to ensure that all staff using the ACC completes the e-learning training before they use the ACC
- From time to time organisations will be asked for
People with learning disabilities & dementia and those with other health conditions can be:
- Less likely to participate in National Screening Programmes
- Misunderstood as people see the disability/condition and not the person
- More likely to have their health needs ignored by some health professionals
- More likely to face barriers when accessing health care services
