This showcase event presented the innovative and successful Cardiac Rehabilitation and Ainsdale Medical Centre lipids project, integrating a primary and secondary care lipid pathway and management for patients with cardiovascular disease.
The pilot study involves the Cardiac Rehabilitation Centre, Liverpool Heart and Chest Hospital NHS Foundation Trust and Ainsdale Medical Practice. The team of healthcare staff including Nurses, Pharmacists, GPs, Cardiologist, Lipidologist with support from the Innovation Agency have designed a lipid pathway to deliver a high quality of lipid care to patients with cardiovascular diseases.
The meeting highlighted the innovations brought into lipid management through the care pathway and its impact on patient care.
This is an educational session, designed to:
a. Support colleagues to optimise patients' lipid management by implementing the pathway
b. Showcase innovative work which could be replicated in other areas
c. Educate the new NICE lipids pathway and statin intolerance pathway
d. Discuss how we can work together to improve patient outcomes
e. Spread good practice across Cheshire and Merseyside
