In 2020, UCLPartners developed a series of Proactive Care Frameworks with the aim of helping people who are living with long term conditions stay well both during COVID-19 and in supporting the system to adopt a different approach to managing long term conditions, which focussed on:

  • Stratifying and prioritising patients using clinically valid tools – identifying those most in need
  • Optimising the wider practice workforce
  • Utilise digital resources
  • Personalise care and encourage self-management

These frameworks are now recommended by the Royal College of GPs as being helpful in supporting GP practices with recovery and in addressing health inequalities.

The Innovation Agency will present findings from the implementation of UCLP’s Proactive Care @Home and BP @Home framework within the North West Coast.

In this webinar, members of the Innovation Agency team and clinical stakeholders will showcase some key areas of work that have been undertaken in the region. This includes work involving the BP framework, the asthma framework and a “one stop” approach, incorporating several of the frameworks, tailored to the local population to address multi-morbidity. We will focus on how they were applied in GP practices and the impact they had for patients.

These UCLP frameworks focus on Hypertension, Type 2 Diabetes, Cholesterol & FH, Atrial Fibrillation, Asthma, and Chronic Obstructive Pulmonary Disease and have been widely used across the country.
